Application Process

For Departments:

• Upon making an initial job offer, fill out the Department Endorsement Form and email to Medical Staff Services

  • This must be filled out before the credentialing application can be made available to the applicant

• Have the applicant complete a privileging document and controlled substance prescribing delegation form (if applicable)

• Complete the identity verification and process the Identity Verification Form

• Work with Human Resources to complete hiring process

For Applicants:

• You will be receiving an auto-generated email today from evalAppCentral@CactusSoftware.com with a link to our credentialing application

• You will need to create an account/password to access the application. Please retain this information for continued access to the application system

• Please complete the credentialing application within seven days of receipt of this email

• Detailed instructions on how to fill out the application are available through the “Help” feature in the upper right hand corner of the application

• Subsequent reminders and/or communications will also come from this email address. Please add this email address to your contact list to avoid future communication going into your Junk/Spam folder

Some additional important information for the Applicant:

• If you will be prescribing controlled substances and do not yet have a Michigan DEA registration, apply now (please reach out to your hiring department for instructions on applying)

• If you will not be prescribing controlled substances, you must complete the DEA waiver found under “My Documents” in AppCentral

• If you do not have an NPI number, you must apply for one online through the NPPES website: https://nppes.cms.hhs.gov/#/. Create an account and apply as an individual provider
