A 48-year-old homemaker is seen because of two episodes of palpitations and dyspnea. Both were nocturnal, awakened her from sleep, and caused severe anxiety and orthopnea. (Duration 30 to 40 minutes). Physical Examination: BP 106/70, P 76+reg., R 14. Examination negative except that the following was heard on auscultation.
