A 58-year-old woman is admitted to the hospital for gynecologic surgery for uterine cancer. She is seen because a preop EKG shows incomplete right bundle branch block. She denies all cardiovascular symptoms except mild fatigue. Physical Examination: BP 110/70. P 74+reg. R 12. Normal carotid and jugular pulses. No cardiac enlargement. Mild but definite systolic outward movement over mid-precordium. The following was heard on auscultation
