Application & Instructions
Information for Canadian and Osteopathic Students
Only complete applications will be processed. A complete application includes:
- A completed UMMS web-based application with the home school's signature of approval
- An official medical school transcript
- A completed UMMS immunization record . Documentation of a TB test administered within one year of the anticipated elective(s) must be included.
- A processing fee of $100 (refundable if elective is full or canceled). Make checks payable to the University of Michigan .
Instructions:
- Complete the online web application and click 'Submit Application." Your application must include a headshot image file with a maximum size of 50k
- Print the application and have it signed by your school official
- Print the Immunization Record and have it completed and signed by a health care professional.
- Mail the signed application, immunization record and transcript (this may need to be mailed under separate cover) to:

Incomplete applications will not be reviewed so be sure to submit all required documents together. Please allow at least 8-10 weeks for processing.
