Application & Instructions
Only complete applications will be processed. A complete application includes:
- The UMMS application with the home school official's signature of approval
- Official medical school transcript
- UMMS Immunization record [doc] completed by a health professional; please do not attach personal immunization records. A TB test must be administered within one year of your anticipated elective(s).
- Registration fee of $100 (refundable if elective is full or canceled). Make checks payable to the University of Michigan. In-state students (MSU & WSU) are exempt from this fee.
Instructions
- Complete the online web application and click "Submit Application"
NOTE - You must include a headshot image file with maximum size of 50K in order to submit your application - Print the completed application, and have it signed by your school official
- Mail complete application to:
