Sub-Acute and Long Term Care

The Geriatrics Center and the U-M Health System participate with several local skilled medical facilities to help patients transition from the hospital into skilled care settings. Our partnership allows this transition to proceed more quickly and with less complications. The results provide patients with shorter hospital stays and fewer return hospitalizations during recovery.

U-M geriatric physicians and nurse practitioners provide expert continuity of care for patients who no longer require hospitalization, but who still need skilled medical care in a transitional setting. The area facilities currently partnering in the program are: Glacier Hills Skilled Nursing Center, Heartland Health Care Centers of Ann Arbor and Canton, and Superior Woods Healthcare.

Arbor Hospice & Home Care

The Geriatrics Center and University of Michigan Health System participate in a preferred provider relationship with Arbor Hospice & Home Care. As a non-profit and Medicare-certified organization, Arbor Hospice, headquartered in Ann Arbor, MI, provides home and hospice care to seriously ill patients of all ages and their families throughout southeastern Michigan. Arbor Hospice is committed to enabling individuals to complete their lives with dignity, in comfort, and in peace. Staffed by faculty and fellows, from the Division of Geriatric Medicine, total care is provided by a team of over 200 professionals in patient homes, hospitals, nursing homes, assisted living facilities and in their 30-bed Arbor Hospice Residence in Ann Arbor.
